Our priority is supporting our clients to access our services while ensuring NDIS quality and safeguards. 

To ensure a smooth referral process and to gain a better understanding of the participant's goals, please provide as much information as possible in the referral form, together with the NDIS plans and therapy goals.

We appreciate that an NDIS plan is a personal document, however by providing a copy of your plan (or your goals and Improved daily living budget) we can provide the most suitable therapy and ensure we are working within the hours available. 

Once your referral has been received, you will receive a Service Agreement outlining hours required, cost and details of therapy services. The Service Agreement can be signed at the first appointment, or signed prior and returned to referrals@rangeshomephysio.com.au
